This information form is to be completed by enrolled MMA students to initiate the license application process. The information gathered here will facilitate efficient processing of your license application and preparation for issuance.
Mariner Reference Number. You will need your 7-digit Mariner Reference Number in order to complete this form. The Mariner Reference Number is easily obtained. Look it up at the USCG National Maritime Center's Credential Verification Page - on that page, scroll down to Name, DOB, SSN search and enter your information - click SEARCH, and then your Reference Number will be listed at the upper-right of the results page.
Seniors applying for their license should hold a Cadet MMC at this point. In rare and exceptional cases, a student might have reached this point without holding a Cadet MMC. If you do NOT hold a Cadet MMC but you are a graduating senior, STOP HERE. Do not complete this form. Make an appointment with CDR Huhnke, Director, Mariner Credentialing, to make sure your application is filled out and processed correctly.
