Massachusetts Maritime Academy - Class of 2018 Information Packet Health Services - Class of 2018 Information Packet

All incoming cadets are required to have a physical examination, submit the dates of receiving the mandatory vaccinations and submit a copy of the front and back of your insurance card.


Physical exams will be provide at the May 17 Welcome Aboard event. There is no out of pocket cost to you for the exam. Your insurance will be billed for a "Fit for Duty" exam but we do not balance bill or collect co-pays or deductibles. You must bring your insurance card. 

Students attending Welcome Aboard must print the Mandatory Vaccinations form (page one of the document found by clicking the "Class of 2018 Physical Form" tab on the left)
 have it completed by your healthcare provider and bring it to Welcome Aboard. If you are not attending Welcome Aboard, you must print all pages have them completed by your healthcare provider. Completed medical forms are due in the Health Services office no later than June 30, 2014.

All students must submit a copy of the front and back of your insurance card by mail or by bringing it with you to Welcome Aboard.

A medical history must be completed on-line once you receive a MMA user name and password. Instructions will be posted on the Health Services web page after the log-in credentials are sent to you.


The following is a list of the mandatory immunizations for you to reference:

Hepatitis A Vaccine: Two doses required to complete the series. At least one dose must be given prior to attending MMA. 

Hepatitis B Vaccine: Completion of the three dose series.

Meningococcal Vaccine: One dose for all residential students given within the last 5 years. If the first dose was given before age 16 a booster dose is required.

MMR Vaccine (Measles, Mumps and Rubella): Two doses or proof of immunity by a blood test

Varicella Vaccine (Chicken Pox); Two doses or documentation of having had the disease

Tdap Vaccine (Tetanus, Diptheria and Acellular Pertussis): One dose required

Mantoux (PPD) test for Tuberculosis: Must be done within one year of starting at MMA. This is required for all students. If you had a BCG vaccine you may have an IGRA blood test.
