Log on to MMA’s On-line Student Health (OSH) Portal found at https://maritime.medicatconnect.com

User name is firstname.lastname (example: john.doe)

Password is lower case first and last initial and student ID# (example: jd123456)

Complete the on-line Medical History found on the left of the screen. You will need your insurance card to complete the insurance questions.

Print out forms found in the Education section of the screen. Have them completed by your healthcare provider. Have the form(s) stamped with the provider’s office information.

* Please note: all listed vaccinations are mandatory for MMA. This includes the Hepatitis A vaccine and the booster dose of Meningitis vaccine (if dose #1 was given before age 16). A skin test for Tuberculosis  called a PPD or Mantoux must be done within one year of starting at MMA.

** Please note: If you wear glasses please bring them to Welcome Aboard for the eye examination. If you wear contacts please bring a case and solution so you can remove them. If you have a physical exam at “Welcome Aboard” your insurance will only be billed for a “Fit for Duty” examination.  We do not balance bill or collect co-pays and deductibles. There will be no out of pocket expense to you.

Mandatory Vaccines include:

Hepatitis A Vaccine: Two doses required to complete the series. At least one dose must be given prior to attending MMA. 

Hepatitis B Vaccine: Completion of the three dose series.

Meningococcal Vaccine: One dose for all residential students given within the last 5 years. If the first dose was given before age 16 a booster dose is required.

MMR Vaccine (Measles, Mumps and Rubella): Two doses or proof of immunity by a blood test

Varicella Vaccine (Chicken Pox); Two doses or documentation of having had the disease

Tdap Vaccine (Tetanus, Diptheria and Acellular Pertussis): One dose required

Mantoux (PPD) test for Tuberculosis: Must be done within one year of starting at MMA. This is required for all students. If you had a BCG vaccine you may have an IGRA blood test.
